HRSN Housing Services

Elevate your family into long term security with OHP Health-Related Social Needs (HRSN) Benefits.

Contact our HRSN Housing Service screeners who will set up an appointment with you to fill out the screening application and submit it for you to IHN/OHP. They will also work with you to create a housing stability plan to make sure this amazing housing benefit helps to elevate your family into having long term security!

Are you currently on OHP and are currently housed, renting or leasing, but are at-rick of losing your housing if a crisis happens? Many of us do not have a cushion to prevent us from losing our stability, but OHP & IHN want to help make sure that you are able to have that stability!

Annual Income Eligibility

Linn County Income Limit

  • Family of 2: $20,400 or less
  • Family of 3: $22,950 or less
  • Family of 4: $25,450 or less

Benton County Income Limit

  • Family of 2: $25,950 or less
  • Family of 3: $29,200 or less
  • Family of 4: $32,400 or less
